I have my signed, completed verification form. Now what?

1. Make a copy of the form for your records.

2. Turn in the original “J. H. Rose High School Volunteer Service Verification Form” to the PVSA box located in Ms. Noles’s room. (MAKE SURE TO KEEP A COPY FOR YOURSELF!)

3. The PVSA coordinator will keep a running total of your qualifying volunteer hours. If you meet the requirements for the PVSA, you will be nominated for the award.

How do I get nominated for the PVSA?
If you have earned 100 or more qualifying volunteer hours from January 1, 2014 to December 31, 2014, and have submitted the proper paperwork by January 31, 2015, you will be nominated for the President’s Volunteer Service Award by J.H. Rose PVSA, an official PVSA certifying organization.

When is the last day to submit my documentation?
The last day to submit verification forms is January 31, 2015.

Will you accept late forms?
No, late forms will not be accepted.

When will the awards be presented?
Awards earned for 2014 will be presented in the spring of 2015.
Awards earned for 2015 will be presented in the spring of 2016.

What will recipients receive?
Award recipients will receive a PVSA pin, a congratulatory letter from the President of the United States, and personalized certificate commemorating their hard work.
